Medicaid Waiver: I Cannot Find the Out Of Network Provider Form, Which is Required of Out of Network Providers Who Continue to Follow Their medicaid clients anywhere on the WHN LME website

I just want to make evident how difficult it is working with WHN LME re: being a Out Of Network Provider. Most of the links on this page no longer work: http://www.westernhighlands.org/provider-enrollment.html#Initial Enrollment Forms. I have already sent in via US post the 9 page Client specific Agreement Form for a Medicaid client along w/ my initial DMA 'Medicaid Provider approval' form. I don't know if you are requiring more than this as I cannot find the Client Specific Agreement Form at the 'search' box at the WHN LME website, as it only allows this much to be typed in: ' client specific agre' and no useful information comes up. Therefore, I cannot even see what is on Page 2 of the Client Specific Agreement Form which is to be used by OON providers. Please advise. thank you. Marsha V. Hammond, PhD
